Did Misato have a Rei visitation? I never saw one. Assuming she did, 
what about anybody outside of NERV? According to your theory, all who 
ever lived and died would be saved, as long as Rei visited them... but 
I recall the spirits coming to her, not the other way around. And in 
that case, some sort of selection process would have to take place.

True, Asuka did reappear. But then again, the TV episodes where made 
before the movie, what happens in the episodes looks much like the 
classic concept of heaven to me. Anno would have made things a lot 
easier if he had stayed there. After all, even if only a fraction of 
all humanity ever reappear, imagine the overpopulation problems they'd 
face - especially as Earth, or at least parts of it, is depicted as a 
barren wasteland!
