I assumed that at the start of the show, Rei suffers from injuries suffered
in the synch test we later were shown, when Gendou ran in to open the plug
and check on Rei.
My assumption at the time of viewing was that either Rei's injuries seen in
the flashback weren't as obvious at the point when Gendou opened the plug
(severe internal injuries, broken bones held in place by the plug-suit,
maybe a later degradation of her condition), or it was just a genuine
failure of continuity - or revisionism - on the part of the writers.
Either way, I expect the melodrama of the scenes is the important thing,
rather than the minutia of the details.

I agree about the melodrama, but Eva is about detail as well.
At the start of 1.01, we see that, different from TV ep.1, the bay is red
from LCL. On top of that is the "body was here" white outline of a giant
figure, also new animation.
I maintain the figure outline & LCL is from Eva 00, after it was beaten by
the angel. That would more than account for Rei's injuries. It took Eva 01
going beserk & instinctive cunning, i.e. becoming as savage as the angel, to
beat it in a "wetworks" brawl.
NERV was still learning about angels at the start. The gentle, but obedient
Rei was sent out in a prototype Eva, that had rejected her before, to try
stop a killing machine. She was beaten, the army stepped in & failed, & then
Shinji arrived & Gendou was ready for his next attack with 01.
Its possible he calculated Rei would fail, as would the army afterwards &
set up the scene for needing Shinji to win.
