[EVA] Evangelion 1.01 R4 (Australia & New Zealand PAL) release

In the original series, Misato was initially a Captain, but was promoted
to Major in episode 12 (they had a party too).  Effectively, this didn't
change the rank structure of Nerv one bit and she didn't outrank anyone
she didn't use to.

In Rebuild, Misato is a "Lieutenant Colonel".

They never definitively established what the full Nerv rank structure is,
given that they're a paramilitary organization loosely operating under the
UN.
